When you receive a letter from the SVB requesting you to upload your proof of life, you must scan the QR code using your smartphone’s camera, which will direct you to the SVB process.
If you receive a message stating that your QR code is no longer valid, unfortunately, you cannot send a digital proof of life to the SVB. You also cannot request a new QR code.
What can you do?
Fill out the enclosed paper proof of life form, have it signed by an authorized institution, and send it to the SVB.
If you have lost the paper proof of life form, call the SVB to request a new one.
